What seems like a simple spaghetti western on the surface is actually a deeper metaphor for his battle with cancer.
The bad guys are the disease, the whores are the cancer treatments, and the final bullet that took him out was a gutshot.
But he didn't give up and die, he got on his horse and rode off into the sunset.
